VLAC Parent/Student Contract Acknowledgement
I, (We), the undersigned parent/legal guardian of the enrollee(s) have read and thoroughly discussed with my (our) son/daughter this VLAC Parent/Student Contract and hereby agree to the terms and conditions as stated. I (We) understand not following the terms and conditions of this Contract may result in disciplinary action which may include removal of my (our) student from the Virtual Learning Academy Consortium.
Technology and Internet Acceptable Use Policy
I (We), the undersigned parent/legal guardian of the enrollee(s) have read and thoroughly discussed with my (our) student the VLAC Technology and Internet Equipment and Acceptable Use Agreement, the terms and conditions of which are incorporated herein by reference, and agree to the terms and conditions as stated. By electronically executing this Agreement, I (we) expressly agree to be responsible for my (our) student’s use of technology in conformance with the Technology Code of Ethics and Rules for Technology Use, and to assume all responsibility for any liability associated with his/her use of the technology. I (We) further understand and agree that Oakland Schools assumes no responsibility for the student’s or my (our) use of the technology. I (We) agree to return all technology equipment issued to my (our) student if the he/she is no longer enrolled in VLAC, or at any time if requested or required by Oakland Schools. In the event any of the technology equipment is not returned, I (We) will be charged and agree to pay the actual replacement costs of the technology equipment to Oakland Schools. I understand that violating these rules may result in the loss of using the technology by my student and that disciplinary action may result which may lead to removal from VLAC.
